You do not need to travel to a clinic to be seen. We bring the assessment, the treatment and the rehab plan to your home, your community hall or your care setting, and we work at the pace your body allows.
Who we are
Gaj Health Care is a community physiotherapy and rehabilitation service run by GAJADHARSINGH LIMITED. We work with adults who find it hard to get to a clinic, whether that is because of mobility, transport, caring duties or simply the distance to the nearest hospital. Our physiotherapists travel to homes, supported living settings and community venues, so the treatment happens where you actually live your day.
Much of our work is rehabilitation after a fall, a stroke, a joint replacement or a long spell in hospital. Some of it is managing a long-term condition such as Parkinson's, arthritis or a chest problem that leaves you short of breath. We also see people who are simply becoming less steady on their feet and want to do something about it before a fall happens.
Every visit starts with listening. We ask what matters to you, not just what is wrong with you. That might be walking to the corner shop again, getting up the stairs without help, or feeling confident enough to go out on your own. From there we build a plan with realistic goals, and we write it down so you, your family and any other professionals involved can all see the same picture.
Our physiotherapists work alongside carers, district nurses, social workers and GP practices, and we are happy to share reports with anyone you would like involved in your care. If equipment would help, such as a walking frame, a perching stool or a rail by the door, we can advise on what to ask for and how to go about it.
You can refer yourself, or a family member, carer or health professional can do it on your behalf. Home visits tend to last longer than a clinic appointment, because we are working in your space and at your pace. Progress is often gradual, and that is expected. What we aim for is steady, measurable improvement that holds up in your own home, not just in a treatment room.
